1. 4 wks.=$48.00, Checks
payable to: A NEW STAR
PERFORMING ARTS ACADEMY, INC.
(A NEW STAR PAA) Private classes are available at
$65.00/hr. Guests may take a
trial class for $12.00/hr., Two classes for the price of one
(Intermediate II/Advanced only). Out of City/State dancers may take a
class for the drop in rate of $17.00/hr.
Afet choreography CD’s will be available for sale.
2. TUITION: Sessions are paid monthly. For example, 4 wks. =$48.00. TUITION
IS DUE THE FIRST CLASS OF THE MONTH.
Tuition is non-refundable. You do
not receive a credit the following month for classes that are missed. If you are unable to make the first class of
the month, you may mail your check to the above address.
3.
ATTENDANCE: Please call me if you are unable to make a
class.
4.
RETURNED CHECK
POLICY: I offer my students the
convenience of paying by check for tuition payments. If a check is returned due to insufficient
funds, there will be a $25.00 service charge added to your account.
5.
COPYRIGHT: Any choreography/notes given out during class
time are not to be used unless you have the expressed permission of Afet.
6.
DRESS CODE: I expect you to wear the proper attire to
class as indicated by the dress code information of the back of this
sheet. You are required to purchase a
hip scarf and veil for Beginning level classes. Higher level classes will require zills,
tambourines, canes, etc.
7.
VISITORS: Visitors are welcome but must view through
the glass windows to the studio.
8.
REGISTRAION
FORM/FEES: There is a one time
yearly registration form/ fee of $35.00 for liability insurance purposes due
every fall. Individual Fee=$35.00. Family Fee=$15.00 each extra for additional
family member.
9.
STUDIO RULES: We must abide by the studio rules at the studio
space in which I rent.
10.
CELLULAR PHONE: Absolutely NO cellular phone calls or text
messaging during class unless you are a medical professional on call.
11.
PERFORMING TROUPE: I have a performing troupe; Afet’s Dancers of
the Desert Sun. If you are invited to
become a troupe member, classes, dress rehearsal, tuition, registration
form/fees, costume fees are MANDATORY.

The following items are acceptable:
